Our Mission Is:
|
| To provide services for people who are coping
with both the short- and long-term effects of any disabling condition, as well as for
individuals dealing with the conditions of poverty. To enable
persons to have alternatives for living, working, and learning in their communities. |
|

How to receive service: |
|
If you think you
need casemanagement services or if you would like more information, call or email us. Our case managers will talk with you about your
service request and will ask you for the following information:
|
 |
your name, address, and phone number |
 |
a description of the disability |
 |
any financial assistance being received and, if so, the
case numbers from Social Security and/or Public Aid |
 |
how you were informed about CAU |
|
|
| A case manager may meet with you in your
home, at your school or rehabilitation program, at another community agency, or in our
offices. A family interview may be scheduled. You are
eligible for our services if: |

you are an individual who has a developmental disability or
if you are the parent, guardian, friend, or relative of a person who has a disability |
|
AND |
 |
you are a resident of the north side of Chicago, or a
village or city in north and northwest Cook County or Lake County, Illinois |
