Casemanager, Chicago Family Casemanagement
|
Seeking individuals capable of interfacing
with staff of numerous agencies providing referral, medical, support, and generic services
to eligible recipients. One year of experience with pregnant women, infants 0 to 1
year of age and their families, or exposure with community health, maternal and child
health care. A good knowledge of the Illinois Department of Human Services, Department of
Public Health, pre-natal care and community support services is preferred. Bachelor's
degree in Behavioral/Social Science, Social Work or a related field. Bilingual
(Spanish/English) or (Arabic/English) required. Competitive salary &

Casemanager, Chicago Family Casemanagement
|
Position requires Master degree in Social
Work with 1 year experience; knowledge of the Department of Public Aid, Department of
Public Health, pre-natal care, community support & generic services is preferred.
Duties: Interview clients individually, in families, or in groups,
assessing their situations, capabilities, & problems, to determine what services are
required to meet their needs; develop & review service plans in consultation with
clients; arrange child care, homemaker service, prenatal care, substance abuse treatment,
job training, counseling, or parenting classes, to prevent more serious problems from
developing; address legal issues such as child abuse & neglect; provide support post
partum depression & parenting classes; develop service plan in consultation with
client; interface with staff of numerous agencies providing referral, medical, support,
& generic services to eligible recipients

RTA Assessor
|
This project assesses eligibility of persons
applying for para-transit passes from the Regional Transportation Authority. Bachelors in
human service field desired. Experience working with/DD, physically disabled, and mobility
training preferred
The Assessor reports directly to the Project Manager regarding on-going
operations on daily assessments to be completed for applicants referred by the RTA.
Additionally, the Assessor is responsible for interfacing with the staff of RTA and
Paratransit carriers.
